I, the undersigned, person responsible for the group, recognize and agree to the terms below for all the participants of my group listed in the "Participants" section (hereinafter the "Participants"):
The Participants use equipment from Écorécréo (Groupe Écorécréo) and the activity involves risks that may cause material loss or injuries. I accept the nature of these risks as part of the activity, and the consequences that may result.
I commit that the Participants will conduct themselves at all times in a responsible and safe manner in order to reduce the risks of injury for themselves and others, and that they will follow safety instructions and staff directions.
I agree to return the equipment in the condition it was given. In case of breakage, loss or non-return, I consent to a charge of up to $100 as compensation, as well as for any damage done to other people or property as a result of the Participants' actions.
I confirm that the Participants are able to take part in the activity. In the event of a specific condition affecting a Participant, I commit to informing the on-site staff before the activity begins.
I acknowledge having informed the Participants of the group, and their parents or guardians for minors, about the risks related to the activity. I confirm that I have authority over all Participants and that I have obtained the consent of the parents/guardians of minors to participate and to sign this form in their name.
By signing, I acknowledge and agree to assume all risks associated with the use of the equipment and to release Écorécréo and its representatives from any and all liability, losses, damages, judgments, claims or proceedings of any kind and any type, whether or not the fault is a result of the negligence of Écorécréo.
I acknowledge that I have read, understood and accepted all of the terms and conditions set forth in the current liability waiver, in my name and in the name of the Participants.
